1. The winner will be drawn on 1-12-2016. A completed questionnaire must have been submitted and received by 18:00 GMT, 1-11-2016, entries received after this time will not be considered.
9. The winner will be notified by email (using details provided at the end of the survey) before 15-12-2016 and must provide a postal address to claim their prize. Subject to the provision of a valid postal address, the prize will be dispatched within 30 days of the closing date.
14. Prize must be claimed by the 31-12-2016 or another winner will be drawn from the list of entries.
